Services
Together with your physician, the Memorial Home Health team creates and individual plan of care for every home health patient specific to his/her needs and desires. The scope of specialty services include:

- Rehabilitation Therapies
- Disease Management and Education
- Medical/Surgical Nursing Services
- Diabetic Education
- IV Therapy
- Enteral Nutrition
These services are offered to recovering, disabled, chronically or terminally ill patients in need of medical, nursing or social treatment and/or assistance with activities of daily living. We offer home health care in the patient’s home, assisted living facility or retirement centers.
As hospital stays decrease, increasing numbers of patients need services when they return home. Patients are able to receive safe and effective care in the comfort of their own home.
Insurance Coverage
Payment for services is covered by Medicare, Medicaid, most private insurances and Workers Compensation as well as self-pay.
