06-20-12 $15 Sports Physicals
Date: 20-Jun-2012 1- 3 p.m., 3 - 5 p.m. or 5 - 7 p.m.
Location: The Herbert-Perna Center for Physical Health
Local physicians and health care professionals will administer sports physicals for local student athletes entering grades 7 - 12. Physicals will be given by appointment during three different two-hour segments. (1 - 3 p.m., 3 - 5 p.m. or 5 - 7 p.m.) First 160 student athletes to register will be able to receive the $15 sports physicals.
**All proceeds will benefit local student athletes**
To register for a $15 sports physical, contact the Memorial Hospital Physical Medicine Department at 419.334.6630, or talk to your school’s athletic department.
IMPORTANT!
All athletes must have pages 1,3 and 4 completed on the Ohio High School Athletic Association Pre-participation Physical Evaluation Form prior to having physical done. Parent signatures are required on pages 1,3, and 4. Examining physician will complete page 2 of the form.
Physical forms are available at high school offices, The Herbert-Perna Center for Physical Health, or online via the Ohio High School Athletic Association Web site.
